ADAPT, an Aberlour service working within Aberdeen City and Aberdeenshire, provides outreach support to children with disabilities and their families within their homes and communities. ADAPT also provides residential short breaks that range from a few hours to full time holiday respite.
Service aims and what we offer
The aim of ADAPT is to develop and provide individualised support services for children and young people affected by disability and their families.
ADAPT achieves this by:
• enabling families to stay together and children and young people to stay within their local area,
• developing individualised support plans,
• encouraging children to have fun and participate in activities,
• offering families and children respite to enjoy new experiences,
• supporting families to manage their child’s behaviour and needs,
• providing additional in-house support and parenting work for families
• providing additional support to children excluded from mainstream education.
Referrals
Referrals to ADAPT come from Abereen City Council and Aberdeenshire Council Social Work.
The criteria for referrals are as follows:
• The child or young person has complex needs, including sensory, physical and/or learning disabilities.
• The child or young person presents challenging behaviour.
• There is a risk of family or placement breakdown.
• The child or young person is in full-time education and under 18 years.
• The child or young person may otherwise be considered for a full-time residential school placement.
• The child or young person has the potential to return to Aberdeen /Aberdeenshire from an out of area residential setting.
Outcomes
This service contributes to the following Scottish Government National Outcomes.
• Our young people are successful learners, confident individuals, effective communicators and responsible citizens.
• Our children have the best start in life and are ready to succeed.
• Our children live longer, healthier lives
• The life chances for children, young people and families at risk have improved.
